Grant recipients must submit a Final Report as soon as possible after completion of their project and no later than December 1st of the year for which the funds were granted. Final Reports that are not received by the deadline may disqualify you from future grant consideration.
Final Reports are submitted online by logging into your account and completing the online Final Report Form using the link below:
After activating the link you will be taken to a Logon Page.
1. Enter your email address and password and click LOG ON
2. Your Dashboard will appear
3. Scroll to Follow-Up Forms for the grant received
4. Click Edit for the Final Report
5. Complete the Final Report
6. Click Submit
